File a HIPAA Privacy Complaint

If you have questions about the treatment you received from me, or any behavioral health provider, you can reach out to the professional Behavioral Health Executive Counsel (BHEC) to learn more about the rules and even file a complaint if that is what feels right.

Additionally, you have the ability to reach out to the Office of the Attorney General's Consumer Protection website.  You can reach out to them any time you feel as though a business has not done what they claim they would.

As a client, you have the right to have access to the content of your confidential therapeutic client record per the Texas Administrative Code §801.48 (c), Texas Health and Safety Code §181.102, and Texas Health and Safety Code §611.0045. Current clients may request your records by contacting me. I require you to sign a formal Release of Information requesting specific records.
Submit a written request via email to adrianna@culturalrefugeatx.com, with the subject line “REQUEST FOR RECORDS.”

Please allow up to two weeks for this request.
